FIS Athletics
We are excited for the upcoming 2021-2022 school year. Check this page for information regarding athletic teams for FIS students.
Physicals:
7th Grade – All incoming 7th grade students need to complete all parts of the physical exam – Part 1 and Part 2.
8th Grade – Returning 8th grade students need to complete all of Part 1 and the Medical History Page of Part 2 (If certain questions are answered “yes” another exam may be required).
ALL STUDENTS – Need to complete the “Sudden Cardiac Arrest Awareness Form”, “Concussion Acknowledgement Form” and the “Athletic Handbook Acknowledgment Form”.
